Driver Eligibility Requirements

Per our insurance policy, all drivers must meet the criteria below. Please confirm each item to continue with your application.

I am between 22 and 70 years oldInsurance policy requires drivers aged 22–70 inclusive.
I have at least 24 months of valid licensed driving experience for the vehicle classMinimum 2 years holding a valid license for the applicable vehicle.
I have no critical violations in the last 10 yearsNo DWI, DUI, drug-related violations, refusal to submit to alcohol/drug testing, felony involving a motor vehicle, manslaughter, or negligent homicide.
I have no more than 1 major violation in the last 3 yearsMajor = racing, hit & run, reckless driving, license suspension for points, driving while suspended, fleeing/eluding, speeding 15+ MPH over limit, unreported multiple licenses, or non-private-passenger accident.
I have no more than 3 minor violations in the last 3 yearsMinor = other moving violations plus defective brakes, defective equipment, or oversize/overweight.
I authorize DTS INC to obtain my Motor Vehicle Record (MVR) at hire and annually thereafterRequired by policy. Any exception requires insurance company approval and endorsement.
